Community Physicians of Indiana

We accept most insurance and will file claims on your behalf when you submit proof of insurance. However, it is your responsibility to make sure Dr. Lader is a contracted provider with your health plan.

Payment for any co-pay, balance, deductible or non-covered charge is expected at the time of service. If you cannot pay the balance, please let us know. We would be happy to make payment arrangements with you.

In the event that you have coverage but cannot provide documentation, payment is due at the time of your visit. After you provide us with your information, we will submit a health insurance claim form to your insurance company indicating that you have paid for your visit and refund any benefits paid by your insurance company to you.

Dr. Lader is a Community Health Network physician, but we can refer you to other facilities for your convenience or to meet the requirements of specific insurance companies.
